CHI Memorial Family Practice Associates provides comprehensive care for children, teens and adults of all ages. Beth Casady, DO, and Michelle Hamilton, FNP, emphasize disease prevention by focusing on treating the whole person – from birth through adolescence and into adulthood. We’re conveniently located to serve children and families who live and work in Spring City and Sale Creek areas and have onsite x-ray.  Our goal is to provide comfortable,  convenient and prompt care to help you get well and stay well. Our providers welcome new patients.

CHI Memorial Family Practice Associates offers DOT exams and cosmetic Botox treatment.
